Community Flu Vaccine Clinic – Oxford High School

HVHD will be hosting a community flu vaccine clinic on Saturday, October 28th from 9:00 am – 12:00 pm at Oxford High School in Oxford, CT (61 Quaker Farms Rd, Oxford, CT 06478). Appointments are recommended but not required. Visit https://hvhd.as.me/community-flu to schedule an appointment.

Bring your completed flu questionnaire, a copy of your insurance card, CT vaccine program eligibility form (if applicable), and completed unaccompanied minor form (if applicable).
