HVHD will be hosting a community flu vaccine clinic on Saturday, October 21st from 9:00 am – 12:00 pm at Sarah Noble Intermediate School in New Milford, CT (25 Sunny Valley Rd, New Milford, CT 06776) . Bring your completed flu questionnaire, a copy of your insurance card, CT vaccine program eligibility form (if applicable), and completed unaccompanied minor form (if applicable).
