Temporary Food License Application
This application is to be used for all temporary food events and concession stands, as well as itinerant food trucks and carts. Please see application for license duration and fees.

Supporting Documents
Please include a copy of the certified food protection manager certificate, your most recent inspection report, and a copy of the license from your licensing health district (if applicable).
Other Information
For itinerant vendors obtaining a 6-month or 12-month license, a pre operational inspection is required ahead of issuing the license. For 1-day events, HVHD reserves the right to perform a day-of inspection. If there are violations that can not be corrected on site, the vendor may be asked to leave that event.
Approval will indicate compliance with CT DPH Requirements; any other permits required from Zoning, Fire Marshal, Inland Wetlands or Building Officials are not ensured by this application’s approval.
