Plan Review Application for Salons, Bodycare, and Tattoo Shops
This application is used for the plan review of all hair salons, nail salons, massage studios, massage therapists, salon service establishments (including eyelashes and aesthetics), waxing studios, and tattoo shops. This will be used for all new establishments, as well as for existing establishments that are moving locations. Please include all chairs, rooms, and services to be provided.

Supporting Documents
License numbers must be provided for all staff members. Please also include a list of proposed services, cleaning procedures, and spec sheets for new equipment. Floor plans showing all equipment, chairs and rooms must be submitted.
Other Information
A change of use form showing the design flow of the existing septic must also be submitted. If property is on sewer, please contact the sewer department. Approval will indicate compliance with CT DPH Requirements; any other permits required from Zoning, Fire Marshal, Inland Wetlands or Building Officials are not ensured by this application’s approval.
