Housatonic Valley Health District offers Sports Physicals to school-aged children. These Sports Physical are performed by a licensed medical professional on an appointment basis only. Each sports physical is $75 and insurance is accepted. Â

 | Q: Is a sports physical the same as an annual physical my child sees their primary care provider for? A: No, a sports physical is not a replacement for an annual physical. An annual physical includes a comprehensive history and physical exam findings, age-appropriate counseling, and more. Q:What should we expect during a sports physical with HVHD? A: There are two parts to a sports physical: 1) a review of the patient’s medical history; 2) the physical exam. The physical exam helps determine whether it is safe for your child to participate in the selected sport. Our APRNs will assess the following during the visit:
Q: What documentation should I bring to the sports physical visit? A: The form from the sports team/organization that needs to be filled out by the provider; a copy of your child’s most recent immunization records; any recent physical exam documents; the parent/guardian’s drivers license, and payment. Q: What is the cost of a sports physical at HVHD?
